ACCELERATOR LINKAGE COVER LOCATED BETWEEN THE BRAKE PEDAL AND THE GAS PEDAL, CAUSED A REAR END CRASH BECAUSE DRIVER'S FOOT CAUGHT ON THE COVER, AND WAS NOT ABLE TO PUSH ON THE BRAKE PEDAL. VAN WAS TOTALLED IN THE CRASH.
