On [XXX] while driving this vehicle owned by the state of Arizona I was involved in a collision at 40 mph. The airbags did not deploy and I sustained serious brakes and internal bleeding. There were no warning lamps.

Reporting for my Mother in law. Confirmed by me several times. First incident occurred while she was driving, dry roads, normal traffic at approximately 65 MPH the front brakes locked up and abruptly bought the car to a stop.
